Required enrollment documents

To make sure your appointment goes smoothly, please complete and submit the necessary enrollment documents before your visit. Submitting these documents in advance allows us to provide timely and efficient care. If you have any questions, feel free to contact our office.

EMG Prep
How should I prepare for an EMG?

Notify your doctor if you are on any blood thinners, have a pace maker or implanted defibrillators.

Do not put any lotions or oils on the skin.

Please wear comfortable clothing that does not obstruct the area that your doctor will be evaluating.

Appointment
What should I prepare prior to my visit?

We ask that you compile a list of medications you are taking as well as any supplements, vitamins or anything else.  Please include other conditions for which you are being treated.  Write down symptoms you’ve been having and for how long.  Write down any trigger symptoms that start and attach of your condition or worsen it.  If it helps you relax and aleviates some of the pressure when coming to our facility for the first time, we encourage you to bring a friend or family member.  We also encourage you to write down any questions in advance related to your condition, possible diagnosis and treatment.

What to Expect
I’ve been scheduled for a neuropsychological assessment. What can I expect?

Individuals undergoing neuropsychological assessments will work with our administrative staff to be scheduled for a clinical interview with a neuropsychologist and a testing session. This session consists of the completion of paper and pencil and/or computer tasks as well as self-report measures. The session time can range from one to four hours and is patient-dependent. The neuropsychologist will construct a battery of tests best suited to assess the individual needs of each patient, including age, medical and psychiatric histories, stamina, sensory perception (i.e., hearing, vision), and cooperation.

After Appointment
I’ve finished my neuropsychological assessment. Now what?

Our patients and their families are always encouraged to follow up with their referring provider for discussion of results and continuing care. For a comprehensive discussion of results and recommendations and to answer any questions, it is also recommended that they meet with their neuropsychologist. Neuropsychological evaluations are not conducted often, typically only once yearly.
